Treatment Description

This surgery is meant to change the size and the shape of the breast through the use of implants. The surgery is usually for women with underdeveloped breasts, women with breast tissue removed or for women who desire larger breasts. Women choose this surgery also to correct breast asymmetry. Breast augmentation can increase fullness and projection of your breasts, improve the balance of your figure and enhance self-confidence.

Incision methods

Inframammary – within the breast fold, very popular because it is concealed in the skin fold below the breast.

Transaxillary – under the arm, this incision is less concealed, a good option for women who do not want any scars on their breasts

Periareolar – around the nipple, this incision offers the benefit of being the most concealed, it might cause some interference with breast feeding

Implant placement

Submuscular placement may reduce the chances of being felt through skin, and it can reduce the chance of scar tissue hardening around your breast during a mammogram. This placement has longer surgery and recovery period and is usually used for very thin women.

Subglanduar placement can make surgery shorter and reduce your recovery time. Implant edges can be more noticeable under your skin. Imaging during a mammogram can also be more difficult.

Implant Types

There are 3 main types of breast implants

  1. Today's silicone implants are a cohesive and viscoelastic silicone gel inside a hermetic silicone shell. After cutting the gel remains in the shell and do not spills into the implant pocket. These implants come in various shapes, sizes and surfaces depending on your desired results. Silicone implants usually offer natural feeling in the body.
  2. Saline implants consist of sterile saline solution covered with an elastomer silicone shell. These implants behave less naturally in the body and they are very well absorbed and expelled by the body in case they leak. Saline implants are used in the USA.
  3. An alternative to implants is fat transfer (lipografting). A fat is extracted mostly from stomach and hips area and then is carefully modified using special technology and injected into the tissues of the breast. Autologous fat grafts are transferred with the aid of micro fat injection. Implant and fat combination is also possible, it is called composite breast augmentation. The technique is particularly suitable for slim women with silicone implants that are visible beneath the skin and can be camouflaged in this way.

Silicone Implants

The most modern implants are filled by a safe silicone cohesive gel. The gel has the unique atribute quality. It creates a single monolithic but not fluid matter, which shapes the breast absolutely naturaly. In the case a capsule is damaged by a sharp item in your body is made a fix matter, which will not fluid anywhere. However, the risk of the rupture of these quality implants is really rare.

Round or anatomical implants?

The round breast implants give upper fullness and make breasts more visible. For very thin ladies they can seem unnatural.

Planning

  1. One week before the operation, do not take any medications containing acetylsalicyclic acid (such as Acylpyrin, Aspirin, Alnagon, Mironal etc.). It might increase bleeding during and after the surgery.
  2. Ideally, one month (minimum two weeks) before the procedure women should stop taking hormonal contraception or replacement in order to prevent thromboembolism.
  3. Consult your GP or your surgeon in Prague regarding other medications you take regularly.
  4. Bring your usual medicine and pills in the original packaging.
  5. Reduce or quit smoking one month before surgery.
  6. No alcohol 24 hours before surgery, no smoking min. 6 hours before surgery.

Before Surgery

You will have personal consultation with your surgeon and pre-op examination. These can take place on the same day as surgery or usually the day before.

Pre-op examination includes EKG, blood presure and blood tests. Surgeon may also ask for mamograph. It may happen that based on your results, surgery cannot be done because of high risk. In this case you pay only the pre-op costs. To prevent this, please, follow all instructions and provide all information asked.

Surgery

Breast augmentation is performed in full anesthesy and usually takes 1 – 2 hours.

After Surgery

You stay at the clinic for one day. The postoperative care involves medicine (analgesic, and if necessary, antibiotics and anti-thromboembolism medicine) and regular checks on the patient's state.

The post-operative period is painful. The wound is very sensitive, rest is of vital importance. The ideal position is on the back, with the body and legs slightly elevated to reduce complications such as bleeding, haematomas or swelling.

After Care

  1. Restrict use of arms for at least 5-7 days, no vigorous activity for 4 weeks.
  2. No lifting above the head for 2 weeks.
  3. Body contact sports avoided for 6-8 weeks.
  4. Social and employment activities can be resumed in 7-10 days.
  5. Cigarettes and alcohol slow down the healing process, they should be avoided (at least one to two months).

Complications

  1. You may experience some numbness of nipples and operative areas, burning sensation in your nipples for about 2 weeks, temporary soreness, tightness, swelling.
  2. Scars may be red and angry looking for 6 months. In time, these usually soften and fade. Most of the discoloration and swelling will subside in 4-6 weeks.
  3. Capsular contracture is a response of the immune system to foreign materials in the human body by creating a tisue capsule around the implant. Consult with our surgence for more details.

Questions & Answers

Breast augmentation is the third most performed plastic operation in the world so it is not exclusively for superwomen. The level of pain obviously varies according to individuals and their pain tolerance. But you do not have to worry. It is normal to feel a little pain after the surgery and your breast can be sensitive and swollen, however the pain will be reduced by medication in the first couple of days after the surgery.
Most of the practitioners received their postgraduate education either in UK, US or Germany. Clinics have international certifications and the state health authorities in countries such as the Czech Republic often require higher standards to obtain the licence to run a medical facility than those required by the EU regulations. This period of time varies. Some patients feel comfortable after 2 days of surgery; some need more time. However it is recommended not to rush the trip back home and stay in a hotel for at least two days after release from a clinic in order to experience the most comfort.
The ability to successfully breast feed varies for all women with or without breast implants. About 25 – 30% of women with breast implants report a certain level of difficulties breast feeding, however there is objectively no direct connection between the implants and such difficulties. Studies have shown that babies that are breast-fed by mothers who have received breast implants are facing the same risks as any other babies.
Not necessarily. A large percentage of women have breast augmentation before having children. However, if one is actively trying to get pregnant or intending to become pregnant in a year or two after surgery, then breast augmentation should be postponed until after pregnancy.
Insertion of implants can cause stretch marks, but this is not a common problem. They are very well healed and practically invisible in the future. Appearance of stretch marks (also known as striae) is more dependent on genetics and life style.
Breast implants are not permanent. They can stay inside your body for up to 20 years. Most women though seek revision for change of size, position, shape or feel of the implant, not for implant failures. Many manufacturers provide a lifetime warranty on their implants.
Women may start driving a car one week after the surgery as long as they are not taking any pain medications.
